Thyroid Profile

TSH is the pituitary’s instruction to the thyroid, and it moves in the OPPOSITE direction to thyroid activity: an underactive thyroid produces a high TSH, an overactive one a low TSH. That inversion is what makes thyroid results feel back-to-front the first time you read them.

Reading the pattern, not the numbers

What this panel is easy to get wrong

  • PREGNANCY HAS ITS OWN RANGES, trimester by trimester. Do not read a pregnancy result against these bands.
  • TSH is not interpretable during or soon after an acute illness, and biotin supplements can distort thyroid immunoassays badly — stop biotin for a few days before the test if your doctor agrees.
  • A single abnormal TSH is usually repeated with free T4 before anything is concluded or started.

Where these ranges come from: Commonly published adult intervals. TSH intervals vary between laboratories and assays more than most tests; pregnancy uses different, trimester-specific ranges.
