Limits of this score. ABCD² does not detect the two things that actually drive early recurrence — carotid stenosis and atrial fibrillation — so a low score must never delay imaging of the vessels or an ECG. Most guidelines now recommend urgent specialist assessment for everyone with a recent TIA regardless of score.
