Wells Score for DVT

Pre-test probability of deep vein thrombosis, to direct D-dimer versus ultrasound.

When to use it. A patient with a painful or swollen leg, before ordering anything.

One point each for the nine clinical features; minus 2 if an alternative diagnosis is at least as likely. Two-level reading: ≥ 2 = DVT likely, ≤ 1 = DVT unlikely.

Limits of this score. A pre-test probability, not a diagnosis. It is not validated in pregnancy, in suspected upper-limb DVT, or in someone already on anticoagulation. 'DVT unlikely' plus a negative D-dimer excludes DVT; 'likely' goes straight to ultrasound whatever the D-dimer shows.
