Use A–aDO₂ when FiO₂ ≥ 0.5, PaO₂ when FiO₂ < 0.5.

Limits of this score. Built for populations, not for individuals — it must never drive a treatment decision for one patient. It uses the WORST value in the first 24 hours of ICU admission, so it cannot be scored on arrival. The mortality figures below come from 1985 cohorts and substantially overstate risk against modern intensive care.
