Kidney Function Test

The kidneys clear waste from the blood. Creatinine is the waste product used to judge how well they are doing it, and eGFR turns that creatinine into an estimate of filtration — which is the number kidney disease is actually staged by.

What this panel is easy to get wrong

  • CREATININE COMES FROM MUSCLE. A muscular person can sit above the range with perfect kidneys, and a frail or elderly person can have substantial kidney impairment with a creatinine that still looks normal. eGFR corrects for some of this; it does not remove it.
  • A single raised creatinine does not diagnose chronic kidney disease. CHRONIC means present for more than three months — one abnormal result needs a repeat.
  • Dehydration, a recent very high-protein meal, heavy exercise and several common medicines all raise creatinine temporarily.
  • eGFR equations are not validated in pregnancy, in acute illness with rapidly changing kidney function, or at the extremes of body size.
