qSOFA below 2 does not exclude sepsis. If you suspect infection and the patient looks unwell, investigate and escalate regardless of this score.
One point each for respiratory rate ≥ 22, altered mentation, and systolic BP ≤ 100 mmHg
Limits of this score. A prognostic flag, not a diagnosis of sepsis and not a screening tool to rule sepsis out — its sensitivity is too low for that, and Surviving Sepsis explicitly advises against using it as a single screen. A qSOFA of 0 in someone you are worried about should not reassure you.
